Brisbane Photoshoot - media consent form
Hello,
From time to time we run professional photography sessions at our programs so we can capture the warmth, connection and joy of Upbeat Arts. These photos and video help us tell our story, welcome new members, thank our volunteers and support the work we do together.
We have a photoshoot planned during North Brisbane Choir rehearsal. Eric will be taking photos between 11:15am to 1:30pm on Wednesday 26 August at Wavell Heights. Taking part is completely your choice.
If you are interested in being involved, we ask that you please make arrangements to stay back later after choir, until 1:30pm that day.
We also ask you to please complete the following short consent form to let us know if you would like to be involved or not. It takes just a couple of minutes and lets us know if you are comfortable with being involved.

This short form is only about the photoshoot on Wednesday 26 August at Wavell Heights. It does not change any photo choices you have already made with Upbeat Arts.
Taking part is completely your choice, and choosing not to take part will not change anything about your place at Upbeat Arts. If you would like help to fill it in, you can ask a staff member, volunteer or someone you trust.
